2: In-Game Name of Player Being Reported
3: Reason of Report & Rule # Broken
4: Time and Date of Rule Being Broken
5: Clear Evidence (Video/Screenshots. For videos the time where the hacks are shown must be stated. We aren't going to watch your whole YouTube video for thirty seconds of hacks.)
